Dr. Fahd Jajeh
M.D., F.A.C.C.
Dr. Jajeh is the founding partner of Lake Heart Specialists. He specializes in interventional cardiology and cardiovascular disease. Dr. Jajeh graduated from Damascus University School of Medicine in Damascus, Syria in 1980. He completed his internship and residency with Cook County Hospital in Chicago, IL in 1984, and later completed his cardiology fellowship in 1987.
Dr. Jajeh is board certified in cardiovascular disease and interventional cardiology.
Dr. Robert Koch

Dr. Koch specializes in cardiovascular disease. He attended Chicago College of Osteopathic Medicine in Hyde Park-Chicago, IL where he received his Doctorate of Osteopathic Medicine in 1990. He completed his internship and residency at Loyola University Chicago Medical Center in 1993. Dr. Koch completed his cardiology fellowship in 1993 at Lutheran General Hospital - University of Chicago in
Park Ridge, IL.
Dr. Koch is board certified in cardiovascular disease, echocardiography, and advanced heart failure and transplant cardiology.
Dr. Maher Nahlawi

Dr. Nahlawi specializes in interventional cardiology and cardiovascular disease. He attended Damascus University School of Medicine in Damascus, Syria and graduated with his Doctorate of Medicine in 1993. He then completed his internship and residency with Cleveland Clinic in 1998 and went on to complete his cardiology fellowship with Northwestern Memorial Hospital in Chicago, IL in 2001. Dr. Nahlawi also completed his interventional cardiology fellowship with Northwestern Memorial Hospital in Chicago in 2007.
Dr. Nahlawi is board certified in nuclear cardiology, cardiovascular disease, and interventional cardiology.
Dr. Charles Jaffe

Dr. Jaffe specializes in cardiovascular disease. He attended Des Moines University, College of Osteopathic Medicine and Surgery and acquired his Doctorate of Osteopathic Medicine in 1997. From there, he completed his internship and residency at Advocate Lutheran General Hospital in Park Ridge, IL in 2000. He also completed his cardiology fellowship at the same hospital in 2003.

Dr. Raymond Chow

Dr. Chow specializes in cardiovascular disease. He received his Doctorate of Medicine from Fienberg School of Medicine - Northwestern University in Chicago, IL in 1997. He completed his internship and residency with University of Illinois College of Medicine in Chicago. Dr. Chow completed his cardiology fellowship with Advocate Illinois Masonic Medical Center in Chicago in 2003.
Dr. Chow is board certified in cardiovascular disease and nuclear cardiology.
Dr. Benjamin Rojas

Dr. Rojas specializes in interventional cardiology and cardiovascular disease. He attended Nova Southeastern University College of Osteopathic Medicine - Our Lady of the Lake University in San Antonio, TX where he received his Doctorate of Osteopathic Medicine in 2002. He then completed his internship at Midwestern University of Chicago College of Osteopathic Medicine in Downers Grove, IL in 2003, and his residency in 2006. He completed his cardiology fellowship with Midwestern University Chicago College of Osteopathic Medicine in 2010.

Jennifer Jensen

Jennifer received her Bachelors of Science in Nursing in 2009 from Olivet Nazarene University. She also went on to receive her Masters of Science M.S., F.N.P. - B.C. from the same University in 2014.
Jennifer is a board certified Family Nurse Practitioner.
