You can now send a message to your doctor, or request an appointment. If you have already created an account, just log in to your Patient Portal.

If you have not yet created an account, follow the steps below:

1. Call either office to give us your email address.

2. An invitation from "FollowMyHealth" will be sent to you via email.

3. Follow the directions given in the email to set up and access your portal account.
